Dallas–Fort Worth Integrative Gynecology: Highland Park, Plano & Southlake Pricing, Concierge OB/GYN Practices and a Pellet Market Audit

Across Dallas–Fort Worth, a cash-pay integrative gynecology intake typically costs $350–$625, follow-ups $185–$315, and concierge retainers $2,000–$4,200 a year (IntegrativeGyn.com metro estimates). DFW is a Tier 2 market, where we estimate a PPO’s out-of-network allowable for 99205 at $403–$509. It is also home to BioTE’s headquarters in Irving, and pellet clinics are unusually dense here: insertions run about $600–$950, or $1,800–$3,800 a year, rarely reimbursed.

01Two cities, one sprawling hormone market

Dallas–Fort Worth is really two metros joined by a ring of fast-growing suburbs, and integrative gynecology is spread across all of them. Three features shape the market.

Concierge OB/GYN culture. Dallas has a long tradition of private, physician-owned gynecology practices serving affluent neighborhoods, and several have moved toward concierge or retainer models: fewer patients, longer visits, direct access to the physician. These practices often combine conventional gynecology, menopause care and integrative add-ons rather than branding themselves as purely functional medicine.

A pellet hub. BioTE Medical, one of the largest hormone pellet training and practice-support companies, is headquartered in Irving. Other pellet networks are active here too. The result is that pellet therapy is offered in an unusually wide range of settings — gynecology, family medicine, med spas and dedicated hormone clinics — and many DFW patients encounter pellets before they encounter any other hormone option.

Corporate benefits. DFW has one of the country’s largest concentrations of corporate headquarters, so a large share of residents carry self-funded employer plans with their own out-of-network rules. Some are generous; others price out-of-network care tightly.

Where practices cluster

  • Dallas core: Highland Park, University Park, Preston Hollow, Uptown, Turtle Creek and Lakewood — the center of concierge gynecology.
  • Collin County: Plano, Frisco, McKinney, Allen and Prosper, with fast-growing hybrid and pellet clinics.
  • Northeast Tarrant and the mid-cities: Southlake, Colleyville, Grapevine, Westlake, Keller, Flower Mound and Coppell.
  • Fort Worth: Tanglewood, Westover Hills, Clearfork and the Near Southside medical district.
  • Irving and Las Colinas: central to both cities and home to many corporate campuses.

Distances are large. A Fort Worth patient seeing a Highland Park practice can spend two hours round trip, so ask how many follow-ups can be virtual.

02DFW pricing benchmarks

These IntegrativeGyn.com metro estimates are built from our 2026 national data and adjusted for DFW. They are benchmarks for comparing quotes, not fees from any named practice. Prices change — confirm in writing.

DFW concierge retainers are often structured as an annual fee paid up front or monthly, sometimes with a separate charge for an initial comprehensive visit. Many practices in the Park Cities and Preston Hollow describe the retainer as covering access and extended time while still billing insurance for covered gynecology services. That hybrid can be a good deal, but you need to know which services are included in the fee and which will be billed to your plan. Read our retainer red-flag audit and compare annual totals, not monthly fees.

Pellet pricing is more competitive here than in coastal metros because the supply of trained providers is large. A lower per-insertion price can still add up once labs, booster visits and supplements are included. If a clinic quotes pellets, ask for a one-year written estimate that lists insertion fees, lab fees, supplement costs and any membership.

For labs, keep standard serum panels on insurance when you can and pay cash only for functional tests that will change a decision; the lab panel cost builder shows the trade-off.

03Pellet market audit: what to check in BioTE’s home metro

Because pellets are so widely available in DFW, the useful question is not “where can I get pellets?” but “what exactly am I buying, and is it the right route for me?” Pellet networks typically train and support independent practitioners, who set their own fees and make their own prescribing decisions. That means the same brand can come with very different pricing and practices from one clinic to the next. Our BioTE vs EvexiPEL vs SottoPelle comparison describes how the major networks differ.

Questions to ask any DFW pellet clinic

  • Who decides my dose, and how? Ask whether dosing follows a proprietary algorithm, the clinician’s judgment or both, and how they adjust if levels run high.
  • What is the all-in first-year cost? Insertions (3–4 a year), labs before and after, any booster or check visits, and any supplements the program expects you to buy.
  • Is testosterone included? Many pellet programs include testosterone. There is no FDA-approved testosterone product for women in the US; evidence supports it mainly for low sexual desire after menopause, and it is a Schedule III controlled substance.
  • What happens if I want to stop? Ask about prepaid packages, refunds and whether you will get a prescription for a non-pellet alternative.
  • Do you offer FDA-approved options? Transdermal estradiol and oral micronized progesterone are bioidentical, FDA-approved and adjustable at any time.

Over three years, a pellet program at $600–$950 per insertion costs roughly $5,400–$11,400 in insertion fees alone, compared with about $900–$3,060 for insured transdermal estradiol and progesterone. Run your numbers in the pellet vs patch calculator and check a specific contract against our pellet clinic pricing audit. 04Out-of-network math for 99205 and 99215 in DFW

DFW is a Tier 2 market. For plans that price out-of-network care from charge data, we estimate allowables at roughly 170–215% of the CMS non-facility national baseline.

CodeCMS national baselineAt 170%At 200%At 215%
99205$236.81$403$474$509
99215$183.50$312$367$395

Table 2 · Source: CMS PFS non-facility national baseline; multipliers and results are IntegrativeGyn.com estimates.

Worked example: Plano patient on a corporate PPO

Your employer’s self-funded PPO pays 80% out-of-network, your deductible is met, and the allowable lands near 200% of the baseline. A Dallas practice bills a $595 intake as 99205 and three $300 follow-ups as 99215.

  • Intake: $236.81 × 2.00 = $473.62 allowed × 80% = $378.90 back.
  • Follow-ups: the 200% figure is $367, above the $300 billed, so each is allowed at $300. $300 × 80% = $240, × 3 = $720.
  • Visits net: $1,495 billed, about $1,099 reimbursed, net about $396.
  • Add four pellet insertions at $750: $3,000, typically $0 reimbursed. Your year now costs about $3,396 — the hormones, not the visits, drive the total.

Model your case in the Tier 2 superbill calculator, and use the superbill and HSA/FSA guide to check coding before you submit.

05BCBSTX and DFW’s corporate payer mix

Blue Cross and Blue Shield of Texas is the largest commercial insurer in the state and administers many self-funded employer plans. UnitedHealthcare, Aetna and Cigna administer a large share of DFW’s corporate plans, and Baylor Scott & White Health Plan also operates in North Texas.

Because so many DFW employers self-fund under federal ERISA law, the employer — not the insurer whose logo is on your card — decides whether you have out-of-network benefits and how the allowed amount is calculated. Some corporate plans pay 70–80% of a charge-based allowable; others pay a modest percentage of Medicare, which can cut the 99205 allowable to roughly $330–$355. Ask your plan which method it uses.

Texas law protects patients from many surprise bills for emergency care and certain out-of-network services at in-network facilities, but those protections do not apply when you knowingly choose an out-of-network office practice. If you pay cash without filing a claim, federal rules entitle you to a good-faith estimate for scheduled care.

If you are on Medicare, ask whether a concierge practice has opted out of Medicare; some DFW concierge practices remain enrolled and bill Medicare for covered services while charging a separate access fee, and others have opted out entirely.

06DFW clinics by model type

We describe organizations only by their publicly known model. We do not publish fees for any named organization, and all prices here are metro benchmarks.

Academic and hospital-based care

UT Southwestern Medical Center in Dallas is the region’s major academic medical center, and Baylor Scott & White, Texas Health Resources, Methodist Health System and Medical City Healthcare run large hospital and physician networks across the metro. These generally bill insurance and are the right starting point for complex histories, including breast cancer, clotting disorders and surgical menopause.

Concierge OB/GYN practices

Concentrated in the Park Cities, Preston Hollow and Southlake, these physician-owned practices offer longer visits and direct access for an annual fee, often while still billing insurance for covered care. Ask each practice exactly what the fee includes and how it handles Medicare.

Integrative and functional practices

Functional and integrative women’s health practices are spread across Collin County, the mid-cities and Fort Worth, often run by gynecologists, family physicians or nurse practitioners under physician delegation.

Hybrid and virtual models

National services — in-network Midi Health and subscription brands such as Winona, Evernow and Alloy — serve Texas patients. Compare models in Midi vs Tia vs Parsley.

Pellet clinics

Pellet therapy is offered across every part of the metro, inside gynecology and family practices, med spas and dedicated hormone clinics, affiliated with several networks. Use the audit questions above wherever you go.

07Texas rules: telemedicine, testosterone and corporate practice

A summary for patients and physicians, not legal advice. Rules change; confirm with counsel.

Telemedicine prescribing

A clinician treating you while you are in Texas generally needs a Texas license. Texas allows a patient relationship to be established through telemedicine when the standard of care is met. Nurse practitioners and physician assistants prescribe under delegation from a physician, and Texas Medical Board rules govern how physicians delegate and supervise procedures — including pellet insertions performed by NPs or PAs in clinics and med spas.

Testosterone

Testosterone is a DEA Schedule III controlled substance. The federal Ryan Haight Act generally requires an in-person medical evaluation before a controlled substance is prescribed via telemedicine; DEA and HHS have extended temporary telemedicine flexibilities several times since 2020 while a permanent framework is finalized. Check DEA’s current notice for the expiration date. Testosterone pellets are typically inserted in person, but testosterone creams prescribed after a video visit fall under these rules. Estradiol and progesterone are not controlled.

Corporate practice of medicine

Texas is a strict corporate-practice state. Lay corporations and investors generally cannot practice medicine or employ physicians, with limited exceptions such as TMB-certified nonprofit health organizations. Hormone clinics and med spas therefore typically pair a physician-owned professional entity with a management company that handles non-clinical services. If you are evaluating a hormone clinic, ask who owns the medical practice and who your prescribing clinician is. Physicians considering a concierge conversion or a pellet license should read our DSC transition blueprint, model it in the practice transition calculator, and confirm structure with Texas health-care counsel.

For the clinical side, see our menopause longevity protocol, and compare with the Austin and Houston hubs. For a second opinion on a pellet or concierge contract, use the matching form. This page is educational and is not individual medical, legal or financial advice.

FAQFrequently asked questions

How much does an integrative gynecologist cost in Dallas–Fort Worth?

IntegrativeGyn.com metro estimates put a DFW cash-pay intake at $350–$625 and follow-ups at $185–$315, with annual concierge retainers of about $2,000–$4,200. Park Cities, Preston Hollow and Southlake practices sit at the top of those ranges; Collin County, the mid-cities and Fort Worth tend to be lower. These are benchmarks, not quotes from any named clinic.

Why are there so many hormone pellet clinics in Dallas?

BioTE Medical, a large pellet training and practice-support company, is headquartered in Irving, and other pellet networks are active in North Texas, so many practitioners in the region offer pellets. Practitioners set their own fees. Pellets are not FDA-approved and are rarely covered by insurance; FDA-approved transdermal estradiol and progesterone are alternatives to discuss with your clinician.

How much do hormone pellets cost in DFW?

We estimate $600–$950 per insertion in DFW, or about $1,800–$3,800 a year at three to four insertions, before labs, booster visits or supplements. Commercial insurers routinely deny pellet insertion and the compounded pellet for menopause. Ask for a written first-year estimate that lists every fee, and compare it with FDA-approved transdermal estradiol, which is usually covered.

Will my corporate PPO reimburse an out-of-network integrative gynecologist in Dallas?

Possibly. Many DFW employers self-fund their plans and set their own out-of-network terms. With charge-based pricing in this Tier 2 market, we estimate the 99205 allowable at about $403–$509, and some corporate plans pay 70–80% of it after the deductible. Plans that pay a percentage of Medicare allow less. Call your plan to confirm.

Is a Dallas concierge OB/GYN retainer reimbursable by insurance?

The retainer or access fee itself is generally not reimbursable, because it pays for availability and services insurance does not cover. Some concierge practices still bill your insurance for covered gynecology visits or issue superbills. Ask what the fee includes, how visits are billed and whether the practice participates in Medicare if you are 65 or older.
